Checklist

Please complete the correct Application for Admission:

If you have never taken a course at Davis College, please complete the New Online Student Application for Admission online.	If you are a returning student and have taken courses at Davis College, please complete the Returning Online Student Application for Admission online.

For students taking less than six credit hours, you will be contacted by the Admissions Office.

For students taking six credit hours or more:

1. Send SAT and/or ACT scores.  Scores are considered official if reported on the official high school transcript or received directly from a testing agency.  Davis College's SAT code is 2233 and the ACT code is 2710.
2. Send Official High School Transcripts or GED.
   1. Home schooled students may submit a letter from their Superintendent of Schools, stating that they have met all requirements of their state's laws for secondary education, or a GED.
3. Send Pastor Recommendation Form.
4. Complete and send Medical Forms from Health Services.
5. Fill out the Free Application for Federal Student Aid.  The FAFSA code for Davis College is 015291.
